Dear Editor

I would like to extend my sincere congratulations on the publication of Clinical Practice Recommendations for Diabetic Foot Attack: “This is How We Do It”, developed by the Latin American Diabetic Foot Association (ALAPID) and published in the Global Wound Care Journal.

This guideline is exceptionally valuable because it is firmly grounded in the epidemiological realities and sociocultural context of the Latin American region, including delayed presentations, access limitations, and the unique patterns of infection and ischemia. By accurately reflecting these regional characteristics, the recommendations offer a practical and realistic approach that is highly relevant to daily clinical practice.
I am also particularly impressed by the detailed descriptions of biomechanics and phase-specific rehabilitation strategies aimed at preserving gait and long-term limb function. Few guidelines provide such clear, actionable methods for restoring safe ambulation, optimising pressure redistribution, and supporting functional recovery.

The integration of the Saint Elian Wound Score System and the structured classification of the five diabetic foot attack types further strengthen the clarity and applicability of the management pathway.

Overall, this publication represents a highly practical, well-structured and clinically impactful guideline that will contribute greatly to improving limb salvage and reducing preventable amputations worldwide.

Please accept my warmest congratulations on this outstanding achievement.
